Application forms must be properly completed. Payment fees are due when you return your forms to the police department for processing.
If you have a middle name you must use the full middle name on line #1. Do not use an initial.
Lines #29 & #30:
You must provide a proper mailing address, including zip code. You must provide a proper telephone number, including area code. Failing to provide sufficient information will result in the return of the application to you NOT PROCESSED and delay the issuance of your permit or identification card.
